We’re seeking a skilled and compassionate Occupational Therapist on a PRN basis to join our home health team. In this role, you’ll provide personalized therapy to help patients recover and regain independence in the comfort of their own homes.

Key Responsibilities:
- Evaluate and develop individualized physical therapy plans.
- Implement therapeutic exercises and interventions to improve mobility.
- Educate patients and caregivers on exercises and safe movement techniques.
- Collaborate with healthcare team to ensure holistic patient care.
Qualifications:
- Current OT licensure.
- Experience in home health or outpatient settings preferred.
- Strong clinical, communication, and problem-solving skills.
- Passionate about delivering patient-centered care.
